This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our commitment
M&D Federal Diagnostics is required by law to maintain the privacy of your protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How we may use and disclose your health information
- Treatment. We may use and disclose your PHI to provide, coordinate, or manage your care and related services, including evaluations and reports.
- Payment. We may use and disclose your PHI to obtain payment or reimbursement for services, including with workers' compensation programs and authorized payers.
- Health care operations. We may use and disclose your PHI for quality, administrative, and operational activities.
- As required by law. We may use and disclose your PHI when required by federal, state, or local law, including for workers' compensation and federal claim programs (such as OWCP/FECA).
- Others involved in your care. With your permission, we may share relevant information with a person you involve in your care.
Other uses and disclosures not described in this notice will be made only with your written authorization, which you may revoke in writing at any time.
Your rights regarding your health information
- Access. You may request to inspect and obtain a copy of your health information.
- Amendment. You may request that we amend health information you believe is incorrect or incomplete.
- Accounting of disclosures. You may request a list of certain disclosures we have made.
- Restrictions. You may request restrictions on certain uses and disclosures.
- Confidential communications. You may request that we communicate with you in a certain way or at a certain location.
- Paper copy. You may request a paper copy of this notice at any time.
- Breach notification. You have the right to be notified if there is a breach of your unsecured PHI.
Our duties
We are required to maintain the privacy of your PHI, provide this notice of our duties and privacy practices, follow the terms of the notice in effect, and notify you following a breach of unsecured PHI. We reserve the right to change this notice and to make the revised notice effective for health information we already have as well as any information we receive in the future.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
