This policy explains how M&D Federal Diagnostics handles information collected through this website.
Effective date: June 11, 2026. This website privacy policy is separate from our HIPAA Notice of Privacy Practices, which governs protected health information created or received in connection with care.
Who we are
M&D Federal Diagnostics ("we," "us," or "our") is a private medical provider serving federal workers' compensation claimants and related OWCP/FECA workflows. We are not a federal agency and are not endorsed by any federal agency.
Information we collect
- Information you submit. When you use a referral or contact form, we collect the details you provide, such as your name, organization, email, phone number, requested service, and any notes you enter.
- Authorization form data. Patient authorization forms are completed through our e-signature provider, DocuSeal. Information entered into those forms is processed by DocuSeal on our behalf.
- Technical data. Like most websites, our hosting provider may automatically log technical information such as IP address, browser type, and pages visited for security and reliability.
- Local storage. We store a small value in your browser to remember whether you have dismissed the patient-form prompt. We do not use advertising or tracking cookies.
How we use information
- To respond to referral and contact requests and coordinate scheduling.
- To provide and process patient authorization forms.
- To operate, secure, and improve the website.
- To meet legal, regulatory, and recordkeeping obligations.
How information is shared
We do not sell your information. We share it only with service providers that help us operate the site and our workflows, including our website host (Cloudflare Pages) and our e-signature provider (DocuSeal), and with others when required by law or with your direction. These providers are expected to protect your information and use it only to provide their services.
Please do not send sensitive information through web forms
The referral and contact forms are intended for administrative and scheduling coordination. Please do not include protected health information, Social Security numbers, or claim documents in a web form. Staff will confirm an approved secure method before any medical records are sent.
Data security and retention
We use reasonable administrative and technical safeguards to protect information submitted through this site. No method of transmission or storage is completely secure. We retain information for as long as needed to fulfill the purposes described here and to meet legal obligations.
Your choices
You may contact the records office to ask about information you have submitted. You can clear your browser's local storage at any time to reset the patient-form prompt.
Children's privacy
This website is not directed to children and we do not knowingly collect information from children through the site.
