Based on your answers, you can go ahead and complete the New Patient paperwork. Please also review the records checklist below so your first appointment is as productive as possible.
New patient records checklist
After completing the paperwork, submit your medical records so our providers can review your history and Department of Labor determinations before making treatment recommendations. Incomplete records may delay your evaluation or require additional appointments.
How to submit records
- Drop off in person, or mail to our office prior to scheduling your initial appointment.
- We are unable to accept records by email or fax.
- Please do not staple, tape, bind, or tab records — everything is scanned, and these delay processing.
- Submit copies only; original records cannot be guaranteed to be returned.
Please provide the following, as applicable
- Your current Accepted Conditions Sheet (ACS) from the U.S. Department of Labor, OWCP.
- All diagnostic studies related to your accepted work injury (no discs or USB).
- All physician records related to your accepted work injury — operative or procedure reports, and emergency room / hospital notes, if applicable.
- Any documentation for prior injuries of the same body parts.
- Schedule award correspondence (e.g. Award of Compensation) if you received a prior Schedule Award.
- Any other documents you feel are pertinent to your claim.
